Unleash your true Potential Program is our Unique AZMADTCARE Participant Program which is designed to provide high-quality support services while embracing family values, collaboration, on your unique journey. Tailored around you, the program helps you achieve your goals and increase your independence, all while being supported by a Team that truly cares for you.
Initial Meeting
We arrange a meeting with you, your family, and advocates to discuss your needs, goals, and preferences. Together with your Support Coordinator, we review your NDIS plan and documents to prepare the right supports.
Service Agreement
Once you choose AZMADTCARE as your provider, we work with you to create a Service Agreement outlining the supports you will receive, the costs involved, and the terms of service.
Support Planning
We complete a detailed assessment with you, your family, and advocates to understand your goals and needs. Together with Allied Health professionals and Support Coordinators, we develop a personalised support plan.
Ongoing Support
Your dedicated AZMADTCARE team delivers your supports and regularly checks in with your Family, Allied Health professionals, and Support Coordinators to ensure services stay aligned with your goals.
